Return Service Authorization Form

The Return Authorization form below is needed for sending parts back to Davis Technologies for Service or Diagnostics. Please fully fill out all the information in the form. You must click on the “Submit” button at the bottom once completed.

Once submitted, the form will be emailed to the email address you provided. There will also be a pop-up that will allow you to open the submitted form and print it out. Please include the form inside your package when you ship your parts in so we know what needs done and whose parts they are.
